You can download and print it as a Microsoft Word document or as an Adobe Acrobat PDF. Fill it out and either mail it back to us or drop it in the collection basket at church. Please do not email it to us. Emailing important and confidential account information is not secure and we do not want to compromise your account information inadvertently!
